You can remove it from the templates under /components/modules/cpanel/ I don't have explicit instructions, but it should be in a file ending in .pdt

I think you're referring to the fields required by the module, which are defined in the module's source code you're using found under /components/modules/MODULENAME/MODULENAME.php.

.. Edit: And of course that the plugin is installed. If it still doesn't work, uninstall and re-install the plugin.
