If you have suggestions on how this would look/work let's open up a discussion. The task doesn't have any real details, and those need to be worked out before it can be implemented.

It would be a simple text area or input box for the customer on the cancelation form.  In the admin side this reason would be best displayed on the scheduled services page I suppose.  Perhaps like this:

I prefer it like this if you was to do down that route this would be nice, but it must be for all cancelled services and not just scheduled services, and if possible force customers who are cancelling to add a reason.
