Sqlstate[Hy093] Error When Adding Company

I am getting this error when I try to create another company in Blesta. 

SQLSTATE[HY093]: Invalid parameter number: number of bound variables does not match number of tokens on line 124

Any ideas as to what might be causing it?

Could be missing files (try re-uploading the files).

and maybe restart mysql server/service
