how to add new forms in V4

hello

i am new to blesta i started 2 weeks ago and i have updated to V4 last night

now i am trying to add new forms but it shows nothing

i even lost the old forms

" Do you have another hosting account you can test this on?" the previous version was working problems started with v4

this is the 2nd time i lose access to admin panel

it shows error404

when i get home i will terminate the account and recreate it again

i will inform you later

same problem

it says "the form was successfully added "but nothing ?

Do you host yourself on a VPS or shared / reseller hosting?

it is installed on a shared hosting but i used reseller's account on the same server

you should also add to the upgrade docs , that the remove files in lib directory expect of init.php
