    Solved, there was an error in the GRANT ALL command. So basically Blesta didnt have permission to create. Thanks, patch running good.

    Just for further check: This is what mysql user used with blesta has: GRANT ALL PRIVILEGES ON db.db TO 'user'@'localhost'

    All met, all green. Server version: 5.5.35-0

    Before the fix , it was complaining about ioncube incompatible. After the fix, the setup started working. After filling up database details, and proceed with install, that error appears (the one in the original message).

    Installing Blesta latest version as of today's date with fix for PHP5.5, getting the following error upon initial install: SQLSTATE[42000]: Syntax error or access violation: 1142 CREATE command denied to user 'removed'@'localhost' for table 'accounts_ach' on line 124 in /removed/public_html/lib/model.php The mysql user has all permissions granted. Just in case.
